Transition is an ongoing planning and development process. Different aspects of the four key transition components (community, education, career, and independent living) are emphasized at different points during the four high school years: Grade 9: Awareness; Grade10: Exploration; Grade 11: Preparation; Grade 12: Participation. Transition helps students and families think about work and life after high school and establishes connections with adult agencies and services.
